EyeMed reports a misdirected member file; Harvard’s older alumni incident has also been added to a breach database.

EyeMed says a file sent to the wrong email address on September 22, 2026 may have contained members’ names, addresses, birth dates, Social Security numbers and member IDs, varying by person. People covered by an EyeMed-managed vision plan should check the official sample notice, review financial and insurance statements, and follow any individual notice about free identity monitoring. The public sample does not specify an enrollment deadline.

Have I Been Pwned added Harvard’s 2025 alumni and fundraising incident on November 3, 2026, listing about 850,000 unique email addresses. Harvard’s official notice advises alumni, donors and others connected to the university to verify suspicious communications; it says the affected systems do not generally contain passwords, Social Security numbers or financial account numbers.

California’s breach registry is not exhaustive coverage of the United States.
