Microsoft reports a partial fix for USB audio problems and fixes for File History backups and some app crashes.
As of November 4, 2026, some USB Audio Class 1.0 devices can still fail to start or produce sound after Windows updates. A September out-of-band update, KB5129195, fixed the 8-channel and 3D audio symptoms on Windows 11 24H2 and 25H2, but Microsoft says other symptoms remain under investigation.
Microsoft says KB5124010 and later updates fix a File History problem that could prevent backups from being created or updated on 24H2 and 25H2. It also says KB5127648 and later updates fix crashes in some applications using Windows AC-3 audio decoding on 24H2, 25H2 and 26H2.
A separate black-screen-after-sign-in issue was primarily observed in Azure Virtual Desktop environments and is listed as fixed, rather than as a broad home-PC failure. See Microsoft's 25H2 and 26H2 issue pages for current details.
